The public form collects contact information, ZIP code, topic, and communication consent only.

Schedule when available

The office confirms availability, service status, and what information can be shared through the proper channel.

Clinician visit

A licensed clinician reviews the concern, history, risks, and appropriate next steps during the actual care process.

Step 5

Clear plan

Patients should know what is being evaluated, what is not yet known, and what happens next before treatment decisions are made.

App-supported tracking

When ready, the MWI App is planned to help with education, reminders, organization, and follow-through.
